This service provides homebound patient/s skilled nursing care like:

Disease management for patients with Diabetes, Congestive Heart Failure, Cancer and other diseases.  As well as assistance of care with their personal needs are provided by our Certified Nursing Assistant.

 

Rehab therapy, exercises and medical equipment needs are rendered by the Physical, Occupational and Speech therapists that will promote patient’s rehabilitation towards independence at home and the community.

 

Community resources and other family socio/financial related needs are tackled and handled by our Medical Social Workers making patient at ease thereby helping them to focus more on their medical treatment plan.
